What this policy covers

This policy explains what information spacompliance.com collects through this website, how we use it, and the choices you have. It applies to this website only. It does not cover information handled under a separate services agreement, which is governed by that agreement and by applicable health privacy law.

Information we collect

Information you give us. When you submit the consultation form we collect your name, practice name, email address, telephone number if provided, the state you operate in, and the details you choose to include about your services and current oversight arrangements.

Information collected automatically. Our web server records standard request data including IP address, browser type, pages requested and referring page. We use this for security and to understand which pages are useful.

We do not use advertising trackers or third-party analytics cookies on this site. The only external resource this site loads is a web font service.

Please do not send patient information

This website is not a secure channel for protected health information. Do not include patient names, medical records, photographs of patients, or any other identifiable patient information in the contact form or in email to us. If you need to share clinical records as part of an engagement, we will provide a secure method.

How we use information

We use what you send to respond to your enquiry, prepare an assessment of your compliance position, and, if we work together, to deliver the services agreed. We do not sell personal information, and we do not share it with third parties except where we use a service provider to operate this site or send email on our behalf, or where disclosure is required by law.

Retention

Enquiries are retained for as long as needed to respond and to keep a record of our business communications, then deleted. Records created during an engagement are retained as required by the engagement and by applicable law.

Your choices

You may ask us to provide a copy of the information we hold about you, correct it, or delete it, by writing to info@spacompliance.com. Residents of some states have additional rights under state privacy law; tell us which state you are in and we will apply those rights.

Security

This site is served over TLS and form submissions are transmitted encrypted. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Changes

We will post any change to this policy on this page and update the date above.
